This is the label the prompt is given in the form. It will be viewed by the audit user, and should give them a clear idea of what they’re expected to add here. This is required, and you will not be able to add a Text question to the checklist if it is left empty.

Choose either a Dropdown list or a Checkbox list.

This allows users to choose one option from a list of many.

This allows users to tick the options that apply from a list of many.

Check this in order to change the layout to match a horizontal layout. If you select this option, the label will be on the left of the dropdown list. If you do not select this option, the label will show above the dropdown list.

List the options for your dropdown or checkbox list. This is a required field. You must add at least two options.

Some properties or checklist items may need special instructions. For example, specific ways to add the question, or where to look for the answer. Add those here. If this is left blank, it will not be shown on the form.
